Vital records | Birth and death certificates

Birth certificates are available for people born in the State of Missouri from 1920 to the present. A request form needs to be completed and there is a charge of $15.00 per copy.

Death certificates are available for people who have died in the State of Missouri from 1980 to the present. A request form needs to be completed and there is a $14.00 charge for the first copy and $11.00 for additional copies of the same person’s certificate.

Some birth and death certificates must be obtained directly from the state, but our staff can assist you. All requests made by mail must be notarized. Birth and death certificates can only be obtained at Audrain County Health Department if the birth or death took place in the state of Missouri. For out-of-state births or deaths, please visit the vital records website for that state.

Everyone requesting a birth or death certificate must show some form of identification, a driver's license is preferred, however, if no driver’s license is available, we have a list of other documents, which may be used. Please contact ACHD for the list.

Persons who can request a record include the individual on the certificate, a wife, husband, mother, father, son, daughter, brother, sister, grandparents, grandchildren, aunt, uncle, niece, or nephew. Those who cannot request a record include cousins and a father who is not on the official record (or any of his family members). Most records are available for adopted individuals if that person was born in the State of Missouri. If the adopted person was not born in the State of Missouri, then the Missouri Department of Health and Senior Services should be contacted.
